Forming plural when singular is dictated
The singular form of a word is frequently dictated, when the plural form is correct. Transcribe the correct plural form when indicated.

When a diagnosis is unclear, a dictator may refer to the differential diagnosis. Although the list may contain more than one possible diagnoses, the term always takes the singular form.
The differential diagnosis is dementia versus early Alzheimer disease.
